Location Indicator

RI score data point colors are based on the patient’s unit location at the time of that RI score. As part of the setup configuration, the organization indicates which units qualify as ICU units, which qualify as intermediate care units, which are regular medical/surgical units, and which are emergency departments. RI score data points appear as red dots for ICU locations, blue dots for intermediate care locations, black
dots for medical/surgical locations, and tan dots as emergency departments. This provides an easy visual indicator on the RI Trend graph of the patient’s transitions in level of care.

Tooltip

Hovering over a specific RI point will display a tooltip that includes: 

• Date and time stamp of the RI score
• Location of the patients at associated RI point's date/time 
• RI value of data point selected 

Drill Down Table

When a data point is selected, it is highlighted in green on the graph. Clicking on a RI data point launches the RI Drill Down table to the right of the graph. Multiple data points can be selected for viewing and comparing clinical data. The Drill Down table will display Information in chronological order even If the data points are selected out of order. If 3 or more data points are selected, a scroll bar is added to the bottom of the table columns to enable access to the additional RI score details. Depending on the browser, the scroll bar may only be visible while scrolling. 

In order to clear the selection, the end user can click the “x” on the top right corner of the panel or manually deselect the data point(s) highlighted in green. The drill down table includes information relevant to the RI score including: 

• RI Date/ Time Stamp
• Unit (patients location at time of score)
• RI Scores
• Vital Signs
• Nursing Assessments (includes Braden, BradenQ, BradenQD)
• Diagnostics (Laboratory results) 
• Last RI Updated Date/Time Stamp

It is important to note: 

• Temperature display can be configured based on organizational preference. Configuration options include Celsius only, Fahrenheit only and Celsius/Fahrenheit displayed together in the Drill Down Table. 
• The configuration to display Braden, BradenQ, and/or BradenQD in the Drill Down Table is based on the age parameters determined by the organization. 
• Cardiac Rhythm is not used in the RI Trend calculator once the value is greater than 48 hours old. Once the value is greater than 48 hours old, a value of "--" will display in the Drill Down table.

Shading 

The RI score input fields (i.e., fields other than the unit, date/time fields, and actual RI score) are shaded based on research-derived excess mortality risk-levels associated with deviation of values from normality.

• The white shaded cells indicate the value is associated with less than 10% excess risk. 
• Cells that are shaded light pink are associated with excess risk from 10% to 30%. 
• Medium pink shading is used for values associated with excess risk from 30% to 50%. 
• Dark pink is used for values in excess risk greater than 50%. 

Hence, the darker the pink, the greater the risk associated with that particular value of the variable, and, therefore the greater an impact that variable has on the corresponding RI score. The pink shading facilitates easy identification of the variables that impact the RI score. The shading is a visual cue only and has no impact on the actual RI score computation. 

Because not all input variables are documented at the same time, values that are new information at the time of the RI point being created are displayed in bold type. Non-bolded variables therefore reflect “carried-forward” data, captured at a previous RI date/time.

Category Tooltip

When hovering over the individual categories in the Drill Down Table, the end user will visualize the expected parameters for each category. The information contained in the Tool Tip will provide insight into the RI calculation and its determination of "abnormal" or "normal." The tooltip will display a representative definition of the input variable.

These are generic definitions and are only meant to provide a general reference to the user who may be unfamiliar with the meaning of the input variable (i.e., these definitions do not consider the specific documentation practices and nomenclature used by hospitals to determine what "normal" or "abnormal" ranges are for that category, but will provide the general criteria used to determine this assessment has
been “met”).

Historical Documentation List

Clicking on any of the input variable category names in the Drill Down table will display a pop-up window showing all of the documentation history (most recent at the top) related to that RI input variable over the course of the patient’s stay. 

For example, clicking on the “cardiac” variable name displays the date/time and associated cardiac documentation for the patient. This will include all the documentation subcomponents that are collectively used to determine if the assessment was deemed “met” or “not met.”

Within the History Table, “not met” clinical details will be bolded in red font for visualization purposes, whereas “met” clinical detail will be visualized in regular font. Selecting the “Close” button will exit the History Table.
